The Supported Living Supervisor oversees the day-to-day operations of residential homes supporting individuals with intellectual and developmental disabilities. This role ensures the implementation of high-quality, person-centered services and compliance with state and federal regulations. The Supported Living Supervisor staff coordinates daily schedules, manages resources, and supports the individuals served in achieving their goals for independence, dignity, and quality of life.
Qualifications:
- Minimum of one year of experience in the field of developmental disabilities or a related area.
- Previous supervisory experience preferred.
- High school diploma or equivalent (required); associate or bachelor’s degree in a related field (preferred).
- Valid driver’s license and reliable personal transportation.
- Successful completion of criminal background checks.
- Ability to complete all SGHS training requirements, including CPR/First Aid, Medication Administration, and Person-Centered Planning.
- Daily Operations:
- Ensure the home operates efficiently and meets all regulatory and licensure standards.
- Oversee scheduling, including shift coverage and time-off requests for staff.
- Monitor household supplies, equipment, and budgets, ensuring resources are used effectively.
- Staff Supervision and Training:
- Supervise and support Direct Support Professionals (DSPs) to ensure effective implementation of Individual Support Plans (ISPs).
- Conduct staff training, including onboarding new team members and ongoing in-service training.
- Provide performance feedback and complete regular evaluations for staff.
- Ensure staff compliance with all SGHS policies and procedures.
- Person-Centered Support:
- Ensure individuals served have access to meaningful activities, opportunities for skill development, and community integration.
- Support individuals in achieving the outcomes outlined in their ISPs.
- Foster an environment of respect, dignity, and choice for all individuals served.
- Assist individuals with daily living tasks as needed, including personal care, transportation, and household chores.
- Health and Safety:
- Monitor the health and well-being of individuals served, ensuring all medical appointments, medications, and treatments are managed appropriately.
- Maintain accurate and timely documentation of health-related activities, including Medication Administration Records (MARs).
- Ensure the home is clean, safe, and maintained in compliance with state and SGHS standards.
- Address emergencies promptly and report incidents according to SGHS protocols.
- Documentation and Compliance:
- Maintain accurate records, including daily logs, incident reports, financial records, and progress notes.
- Conduct regular audits of documentation to ensure compliance with state, federal, and organizational requirements.
- Submit all required reports to the Program Coordinator in a timely manner.
- Communication and Team Coordination:
- Act as a liaison between individuals, families, staff, and interdisciplinary team members.
- Participate in team meetings to review and update ISPs and ensure cohesive support for individuals served.
- Address and resolve conflicts or concerns involving individuals, staff, or families.
- Additional Duties:
- Participate in on-call rotations to address after-hours emergencies.
- Provide direct care during staff shortages or emergencies.
- Perform other duties as assigned by the Program Coordinator or Executive Director.
- Strong organizational and time-management skills.
- Effective leadership and team-building capabilities.
- Excellent communication skills, both verbal and written.
- Ability to adapt to changing situations and resolve conflicts professionally.
- Proficiency in basic computer applications (e.g., Microsoft Word, Excel, and documentation systems).
